What is HARP? 

Health and Recovery Plans (HARPs) are Medicaid Managed Care plans that coordinate and pay for your physical and behavioral health care. Most individuals with HARPs are assigned a Health Home Care Manager to make sure their care meets their needs. HARP members are also encouraged to enroll in Community Oriented Recovery and Empowerment Services (CORE). 

What are Community Oriented Recovery and Empowerment Services (CORE)? 

Community Oriented Recovery and Empowerment (CORE) Services are person- centered, recovery oriented, mobile behavior health supports intended to build skills and self-efficacy that promote and facilitate community participation and independence.

Individuals can receive these services in their homes or out in the community from our dedicated staff members. CORE services provide support for people living with mental illness by establishing goals related to improving social life, employment inquiries or education.

CORE services are available for people 21 and over who are enrolled in a Medicaid Managed Care Health and Recovery Plan (HARP) and found eligible after a short assessment.

What Compeer Rochester Can Provide: 

Psychosocial Rehabilitation

Psychosocial Rehabilitation is designed to assist an individual in improving their functional abilities to the greatest degree possible in settings where they live, work, learn and socialize.

Services will help:

  • Encourage participation in recovery-oriented activities to support and restore social and interpersonal skills

  • Establish support networks

  • Increase community awareness

  • Develop coping strategies for effective functioning at home, work, and school

  • Strengthen community living skills and support community integration

  • Support the identification and pursuit of personal interests

  • Identify and connect with natural supports and recovery resources, including family, community, and faith-based

  • Develop stress management skills

  • Provide rehabilitation counseling and support to implement skills to remain in a natural community setting

  • Assist in acquiring, retaining and improving skills such as communication, self-help, domestic, self-care, socialization, fine and gross motor skills, mobility, personal adjustment, relationship development, use of community resources and adaptive skills necessary to reside successfully in home and community-based settings

Peer Support

Sometimes it is easiest to learn from individuals who share similar experiences. We hire certified Peer Support Specialists to empowerment within themselves to cope with mental health diagnosis. These services include:

  • Non-clinical interventions delivered by peers with lived experience to promote long-term recovery

  • Connections to natural supports in the community to be accessed long after formal services have ended

  • Advocacy, coaching, and companionship through transitions across systems

Family Support and Training

If requested by a client, we work with family members to provide training and support to facilitate engagement and active participation of the family in the treatment planning process. These services include:

  • Training on treatment regimens including recovery support options, recovery concepts and medication education

  • Assist family in providing a safe and supportive environment in the home and community

  • Facilitate support groups under the direction of a certified peer

  • Provide family mediation and conflict resolution services

  • Develop specific problem-solving skills, coping mechanisms, and strategies specific to the individual’s symptom/behavior management and prevention of relapse

  • Collaborate with the family and caregivers to develop positive interventions to address specific presenting issues
